This is a way to gather information, discuss your needs, and determine whether I will be the best fit for you. Insurance eligibility will be reviewed and I will verify your insurance and follow up with you regarding your financial responsibilities.
Step 2
Complete Forms
Once we schedule your first appointment, you will receive an email to access the intake forms (e.g. intake questionnaire, consent, policies) via the client portal.
Complete the intake forms prior to your first appointment; this should take about 15 minutes.
Step 3
On the day of your appointment
For in office therapy appointments:
I am located on the 2nd floor at 7459 Old Hickory Drive.
It is easiest to park in the back of the building and enter in the back door. From there, just head up the stairs in front of you, turn left at the top of the stairs through the door and the waiting area will be the second door on the right. I will come meet you at the start of your appointment.
For telehealth appointments:
Log in to the telehealth video platform where your therapy session will take place to meet at the time of your appointment. You will receive a reminder with a link 24 hours before your scheduled appointment time.
Insurance
New Season Counseling & Consulting is currently an in-network provider with Anthem Blue Cross Blue Shield, Cigna, Aetna, Optima Behavior Health, and UBH.
Services may be covered in full or in part by your health insurance or employee benefit plan. The amount you pay per therapy session depends on if you will be paying out pocket or utilizing your Health insurance and determining what your office visit copay or deductible is.
